COP - Effective discharge planning To provide an informal network for the sharing of knowledge, skills and experience in discharge planning. To stimulate debate around what works best and in what circumstances. To encourage the mutual support and development of individuals within the Community by the Community.
Mental Capacity Act Community of Practice The site is for people involved in implementing the Act in Wales. It contains a wide range of information from the Welsh Assembly Government and links to further information provided by the Ministry of Justice and Department of Health
Preoperative Assessment Development To develop an all Wales process for preoperative assessment with attendant good practice guidelines
CCM Generic Pathway To provide targeted support to all LHBs in Wales on the implementation and further development of their CCM action plans.
Discharge Liaison Forum Providing an opportunity to network, share good practice, problem solve, raise the profile of the discharge liaison role and produce discussion papers, guidelines etc to advise and support practitioners, managers and policy makers.

All Wales Trusts Volunteering Network A network of NHS Trust officers who have responsibility for Volunteering for Health & Social Care initiatives within their Trusts was established in 2007. The aim of the network is to increasing volunteering in secondary care in Wales through sharing good practice and supporting collaborative working with the voluntary sector.
NHS PAs for Excellence The aim of the network is provide a supporttive environment for development, networking and gaining additional knowledge and skills; this will be provided through a range of media such as development sessions, a website,email bulletins, regular network sessions and much more
