This report summarises the learning from the final year of the Chronic Conditions Management (CCM) Demonstrator sites in Cardiff, Carmarthenshire and North Wales.The final testing of the CCM Model and Framework (CCM&F) and the associated evidence based learning will inform Local Health Boards (LHBs) as they speed up changes on the ground and improve services for people with chronic conditions.

During it's running, the Chronic Conditions Management (CCM) Demonstrator Programme was keen to ensure that patients and carers are actively involved in decisions about their care, that they are engaged with across Wales and that their involvement sits at the heart of service planning and delivery. One mechanism to inform this was to interview patients (and carers) themselves and identifying their own personal outcomes and experiences, captured through their personal stories.

The Chronic Conditions Management Demonstrator Programme officially closed at the end of March 2011. There are, however, a number of further Learning Papers and Reports due for completion covering areas such as diabetes, co-production, plus locality and multi-disciplinary team working. Each document will be updated and published throughout June and July 2011 and will be available to view at www.ccmdemonstrators.com

The Cardiff Chronic Conditions Management (CCM) Demonstrator is keen to share its innovate learning within the sphere of CCM with a wide as possible audience. With that in mind a series of roadshows are being held across a number of Cardiff hospital sites from the end of January to the beginning of February 2011.

The Carmarthenshire Chronic Conditions Management (CCM) Demonstrator site has a particular interest in respiratory conditions underlined by the Welsh Assembly's Service Development Directive for Chronic Respiratory Conditions. The work undertaken by Carmarthenshire has seen great strides in health improvement for service users in the locality. The Chronic Conditions Management Demonstrator Programme is currently undertaking a survey throughout December 2010 and January 2011 regarding the work of the Demonstrators and their impact on CCM in Wales.
